New Formulary items:

Rivastigmine (Exelon) patch-centrally acting acetylcholinesterase inhibitor for the treatment of mild to moderate dementia associated with Alzheimer's or Parkinson's disease-available in 4.6mg/24hr patch, 9.5mg/24hr patch

Duloxetine (Cymbalta)-antidepressant -serotonin/ norepinephrine reuptake inhibitor -available as 20mg and 30mg tabs

Fish oil 1000mg capsules

 

New automatic substitution:

Olmesartan (Benicar) 20mg will be substituted with valsartan (Diovan) 80mg

Olmesartan (Benicar) 40mg will be substituted with valsartan (Diovan) 160mg

Omega-3 acid ethyl esther (Lovaza) will be substituted with fish oil 1000mg caps
